Gummy smile

If the upper lip is too short for the bone and teeth it should cover, or the upper jawbone is too long, the patient will show too much gum when they smile, which we call a gummy smile.

The treatment

Treatment for this condition consists in a surgical procedure to move the upper jaw vertically upwards (maxillary impaction) while also moving the lower jaw into position to fit with the new location of the upper jaw.

A gummy smile often goes along with an open bite and mandibular retrognathism.

What is the process for treating Gummy smiles?

First of all, we have to do a proper diagnosis of the type of gummy smile to determine how we will treat it, dentally or skeletally.

If the problem is dental, sometimes it is possible to treat it with orthodontics alone, plus periodontology treatment to reduce the amount of gum covering the teeth, which is often excessive in the case of gummy smiles. This treatment is called a crown lengthening.

If the problem is skeletal and the patient isn’t interested in additional changes to the shape of their face, it can be treated with skeletal orthodontics, using tiny plates and screws.

Finally, if it is a skeletal issue and the gummy smile is severe, we use orthodontics and orthognathic surgery so, if the patient desires, we can improve their facial proportions and lip position. The process for treating a gummy smile is as follows:

What results can we expect from the treatment?

Undoubtedly, the main goal we achieve is correcting the part of the mouth where the gums are exposed when the patients smiles or is at rest.  In women, we recommend showing between 3 and 5 mm at rest and 8 to 10 when smiling. For men we recommend two millimetres less, at rest and when smiling.

Plus, when orthognathic surgery is used, we achieve positive facial changes based on the treatment plan and the desires of the patient.



After the treatment, the patient will have to wear invisible stabilisers at night for a few months.
